The records SAH-SD-007, SAH-SD-008 and SAH-SD-013 call for, one sheet each: the care management activity record that evidences care management claims against the pooled care management account, an End-of-Life budget or the Restorative Care Pathway account, a monthly contact check, the deterioration escalation record, the short-term pathway episode register and care and services plan reviews.
