Document change log

Every change to our documents, newest first, for the aged care and NDIS libraries. When the law or official guidance moves, this is where you see which document changed, when, and why.

Check your version

Every NDIS file name ends in its version, for example CORE-01-08 Whistleblower Policy v1.2.docx. Aged care documents show their version in the grey line under the title, or on a register's READ ME sheet. A higher number is newer.

Aged care: Support at Home and CHSP

The aged care library is stamped to the Aged Care Act 2024 and Aged Care Rules 2025 compilations in force, and every policy and procedure has a compliance mapping that cites the clause behind it. The entries below record the currency work on the launch edition. Version numbers appear here from v1.0.

Version 1.1: second, independent audit of how the law applies

Packs affected: Registration & Monitoring Essentials and Personal Care Provider Library

Every document in the aged care library is now version 1.1. A second audit read each statement of law for how it applies: who must act, what triggers it, the timeframe and how it is counted, and the categories and programs it applies to. It was checked against the Aged Care Act 2024 (Compilation No. 2) and the Aged Care Rules 2025 (Compilation No. 11, read with the amendments that commenced on 1 October 2026), and a separate review checked every change before it was made. 42 of the 59 documents and the READ ME FIRST guide changed; the others carry the new version number only.

Code Area What changed
SAH-SD-003, SAH-SD-002, SAH-GV-012, SAH-FN-008 Ceasing services The written notice under Rules r 149-40, given at least 14 days before the cessation date, now applies to every cessation, including one that follows the participant's own written notice (r 149-35(1)(b)). Template D covers both cases, and the procedure shows how to count the 14 days. A participant who asks to stop keeps only the visits they still want until the cessation date.
SAH-FN-001, SAH-FN-003 Pricing and budget procedures Where the provider holds an unspent care recipient portion the participant agreed it would retain, individual contributions are met from that portion until it reaches zero (r 273A-20(a)). Budgets that itemise personal care are revised for the 0% rate from 1 October 2026 (r 155-50(6) and (7)). The lifetime cap (Act s 273(5)) and the assistive technology and home modifications fee publication duty (r 155-80(8) to (11)) added.
SAH-SG-001, SAH-IM-001 to IM-003, SAH-GV-003 Incidents and safeguarding A participant's choice does not stop the notifications the law requires. Protection for people who report stated as the Act sets it (ss 164(c), 547, 548 and 551). The reportable incident definitions and exclusion in rr 16-5 and 16-10; further information for Priority 2 (r 165A-30(3)); a final report only where the Commissioner requires one (r 165A-45).
SAH-PV-001, SAH-PV-002 Privacy Disclosure decisions now consider the general authorisations in Act s 537 alongside s 168, with the reasoning recorded.
SAH-SD-008, SAH-SD-011, SAH-SD-013, SAH-FN-006 Care management and short-term pathways Care management funding stated as provider-based subsidy in law (Act ss 201 and 203; r 204-5), with the Program Manual's 10 per cent pooled account described as guidance. End-of-Life and Restorative Care Pathway funding conditions (rr 191-5, 195-3 and 196-12) and the separate agreement for an episode alongside ongoing services.
SAH-RT-001 to RT-003, SAH-RT-005 to RT-007 Rights, conduct and suitability Your Rights checked against every right in Act s 23; the responsible person definition (s 12); the annual suitability check (s 172(1)(a)); what a supporter who is also a guardian or attorney can and cannot do (ss 27 to 29).
SAH-SD-002, SAH-SD-010, SAH-FN-002 Late cancellations and no-shows A scheduled service the law treats as delivered (Act s 11A(2); r 11A-5) is always shown on the monthly statement, even where nothing is charged or claimed (r 155-40(4)(m)).

Also amended: category and Standards scoping (r 15-5), worker capability records (Act s 152(1)(d); rr 154-905 and 154-910), citations and cross-references in the other changed documents. Version History tables are unchanged: they hold your own approvals.

Version 1.0: regulatory audit, workability review and two new participant documents

Packs affected: Registration & Monitoring Essentials and Personal Care Provider Library

Every document in the aged care library is now version 1.0. Before release, every statement of law in the 57 existing documents was audited against the Aged Care Act 2024 (Compilation No. 2) and the Aged Care Rules 2025 (Compilation No. 11, read with the amendments that commenced on 1 October 2026), and a second, separate review checked every proposed change before it was made. An operations review read the library the way a provider runs it and closed the gaps between procedures and the records they rely on. The library is now 59 documents, with a READ ME FIRST start-here guide in both packs.

Code Document What changed
SAH-IM-004 If Something Goes Wrong (participant edition) New. The incident management system explained in plain English for participants and supporters, made available under Rules r 164-35(1)(f) and (g), and given with the commencement documents.
SAH-SD-014 Welcome Pack (participant edition) New. A welcome guide for each new participant: contacts, what happens as services start, the documents they receive, visits, safety, rights, complaints and privacy.
SAH-IM-001 Incident Management Policy and Procedure The Priority 1 follow-up notice and the duty to give significant new information (rr 165A-25(6), (7) and 165A-40); the assessment of each incident (r 164-45); SAH-IM-004 given to participants.
SAH-SD-003 Service Commencement and Cessation Procedure The 56 or 84 day funding window and the start notification deadline stated as law (Act ss 92A and 203; rr 92A-5 and 203-11); every start and end tracked on a new commencements and cessations register in SAH-GV-012.
SAH-FN-001 to FN-003 Pricing, claiming and budget procedures The price rules (r 273-15), claiming deadlines (Act s 251; r 251-10), quarterly crediting and the carryover cap (Act s 193; r 193-5) stated as law, with the Program Manual cited as guidance.
SAH-GV-003 Registration Obligations Map A further 25 conditions and duties mapped, each with the document that addresses it, or a plain note where the library has none.
SAH-SD-001 Care and Services Plan Template GP and health contacts, who to contact about a change, and end-of-life wishes, which the escalation procedure (SAH-SD-007) now relies on.
Registers GV-007, GV-011, GV-012, FN-007, FN-008, SD-011, CF-002, IM-003 New sheets for commencements and cessations, lone worker events, the monthly statement run, exits, the care management pool and plan reviews; rating history, review history and Statement of Rights flags.
SAH-README Read Me First: Start Here New. What is in each pack, the order to set the system up, and how the documents work together.

Also amended: every other document, for accuracy of citations, category scoping (r 148-80(2) and (3) apply to delivery under categories 1 to 3; the category 4 and 5 equivalents are in r 15-20) and consistency between documents. Version history tables now start at 1.0, with your own first approval.

Sector review: procedures connected end to end, CHSP options and the evidence map

Packs affected: Registration & Monitoring Essentials and Personal Care Provider Library

A sector reality review read the library the way a provider would use it and looked for the gaps between documents. These amendments connect the intake, commencement, cessation, budget and incident procedures to each other, add CHSP options to the documents a CHSP provider relies on, and rebuild the Standards evidence map against the documents and records in the library. Every legislative statement added was read on the Federal Register of Legislation (Aged Care Act 2024 Compilation No. 2; Aged Care Rules 2025 Compilation No. 11) before it went in.

Code Document What changed
SAH-SD-002 Service Agreement Template A CHSP set of Parts (services, contributions and how they are paid); prior agreement for planned changes to workers, days or times; late cancellation and no-show terms (Act s 11A, Rules r 11A-5); the non-payment ground for ceasing services stated with its full qualifiers (r 149-35(2)(d)).
SAH-SD-003 Service Commencement and Cessation Procedure Commencement checklist and participant receipt; letters for the r 149-40, r 149-45 and r 149-46 notices and for a cooling-off withdrawal; the commencement information required by rr 155-55 and 155-60.
SAH-WF-001 Worker Screening Policy and Procedure The screening test chosen per worker, including CHSP workers at a provider that also delivers Support at Home (rr 152-20 to 152-22); supervision of a worker awaiting a certificate set as a business rule. The register (SAH-WF-002) records the test applied.
SAH-IM-001 Incident Management Policy and Procedure Open disclosure and care plan review built into the incident process, with matching fields in the report form and register (SAH-IM-002, SAH-IM-003); after-hours contact and a deputy set by the business.
SAH-SD-007 Deterioration Recognition and Escalation Procedure After-hours escalation through an on-call contact, or a voicemail and progress note when no one can be reached, matching SAH-SD-004.
SAH-SD-008 Care Management and Care Partner Procedure Care management at least once in each month in which other services are delivered, for ongoing and end-of-life pathway participants (r 148-45); a care partner notice.
SAH-FN-003 Individualised Budget Management Procedure A projected overspend check with an early first-month check replaces the 85 per cent flag; above-budget invoicing conditions applied per service; short-term pathway funding excluded from the quarter-end carryover.
SAH-CL-001 Clinical Governance Framework Scope of practice for support workers drawn from the service list. The Rules prescribe no certificate, so the business sets its minimum qualification.
SAH-GV-004 Change in Circumstances Notification Procedure Each notifiable change described, with what the notice must contain (rr 167-20 to 167-70).
SAH-GV-009 Standards 1 to 4 Audit Preparation and Evidence Map Every outcome mapped to the documents and records that evidence it, with each gap named.

Also amended: CF-001, CF-004, FN-001, FN-002, FN-004, FN-005, GV-001, GV-002, GV-003, GV-005, PV-001, RT-001, RT-003, RT-004, RT-006, SD-001, SD-004, SD-005, SD-006, SD-013, SG-001, WF-002, WF-003 and WF-004: owners for every registration obligation, training aligned to what each policy requires, related documents listed both ways, and citation style made consistent.

Re-stamped to Rules Compilation No. 11

Every stamped document in the aged care library (39 at the time) was re-stamped to the Aged Care Rules 2025 Compilation No. 11 (F2026C00970, compiled 20 September 2026). The changes that commenced on 1 October 2026 are not yet in a compilation, so the documents they affect say that Compilation No. 11 is read with those amendments until a later compilation registers.

Accuracy audit: monthly statements and record keeping

Packs affected: Registration & Monitoring Essentials (GV-002), Personal Care Provider Library (both)

An accuracy audit of two documents against the Aged Care Act 2024 (Compilation No. 2, C2026C00301), the Aged Care Rules 2025 (Compilation No. 10, F2026C00802) and the Privacy Act 1988 (Compilation No. 104, C2026C00227). Every provision was read on the Federal Register of Legislation before the wording changed.

Code Document What changed
SAH-FN-004 Monthly Statement Procedure Short-notice cancellations described correctly (Act s 11A covers services taken to be delivered after a late cancellation, not subscription services). The s 273(2) contribution calculation is explained step by step; the 0 per cent personal care rate is unchanged. The r 155-41 subscription statement duty is now conditional on a subscription service being delivered. The r 155-40(5) statement content summary is completed. Notional accounts defined in full.
SAH-GV-002 Record Keeping and Documentation Policy Retention periods stated per record set instead of a single seven-year rule (r 154-2(2) for prescribed records; separate start points for worker, governing body and advisory body records). The r 154-3 application rule stated. The correction protocol restates Australian Privacy Principle 13.1 in full.

1 October 2026 commencement applied

Packs affected: Registration & Monitoring Essentials (SD-002), Personal Care Provider Library (all seven)

The 1 October 2026 changes to the Aged Care Rules 2025 are in force. F2026L00862 Schedule 4 moved personal care from the independence to the clinical supports means testing category, so the personal care contribution rate is 0 per cent (Rules r 314-10(2) with r 8-105(2); Act ss 7 and 273(2)). Every statement written in the future tense about 1 October was restated as current. No compilation incorporating the change had registered at 2 October 2026, so the documents say that Compilation No. 10 must be read with Schedule 4 until a later compilation registers.

Code Document What changed
SAH-FN-001 Pricing and Invoicing Policy 1 October change restated as in force; compilation reading note.
SAH-FN-003 Individualised Budget Management Procedure Personal care 0 per cent restated as in force; the plain English explainer now reads "Since 1 October 2026".

SAH-SD-002 Service Agreement Template Citation style corrected in two places. No change to content.

  • 93 content amendments from Program Manual v4.3 and v4.4 across FN-001, FN-002, FN-003, FN-004, SD-002, SD-003 and SD-008: the care management carryover cap, the 56 and 84 day funding window, price variation mechanics, Home Care Package unspent funds on exit, the CHSP scope limit on contributions, and the Commission's position on pricing care management into service delivery.

Reporting protections, record retention and Integrity Act accuracy

Packs affected: Core Library, Complete Library, High Intensity, Behaviour Support, ISW Verification, Business Extras, Employer Essentials

A review of documents already in circulation. The changes bring whistleblower and reporting protections, record retention periods and descriptions of the NDIS Amendment (Integrity and Safeguarding) Act 2026 into line with the legislation as it now stands.

Action recommended. If you use the Whistleblower Policy (CORE-01-08), or any of CORE-04-01, CORE-04-02, CORE-05-01, CORE-08-03 or BS-01-02, update your copy. The earlier wording only protected people who reported "in good faith", which is narrower than the NDIS Act.

  • Protection from retaliation no longer depends on whether a report was made in good faith (NDIS Act s 73ZC). The Act also prohibits revealing a discloser's identity except in the circumstances it allows (s 73ZBA).
  • Incident and complaint records: kept 7 years from the day the record is made, and reportable incident records 7 years from the day the NDIS Commission is notified. These periods come from the NDIS (Incident Management and Reportable Incidents) Rules and the NDIS (Complaints Management and Resolution) Rules, not the Practice Standards.
  • Descriptions of the Integrity and Safeguarding Act now match the Act. For example, the unregistered-provider offence applies only to supports the NDIS rules require a registered provider to deliver.
  • Smaller fixes: dates written out in full, two repeated sentences removed and one cross-reference corrected.
